The main findings of this study demonstrated that Physical Therapy has been successful to a high degree in the treatment of signs and symptoms in TMD patients. By comparing each of these signs and symptoms with each other, it was demonstrated the highest effect of Physical Therapy treatments was related to presence of deviation to sides in the path of mouth opening and closing, presence of bulge and asymmetry in masticatory muscles, bruxism and presence of repeated jaw disarticulation in a way that no unrecovered case was left.

Data were analyzed by using the MC-Memar’s test.
The test was applied by a 2×2 contingency table with the dichotomous variable. A count of the individuals was recorded before and after Physical Therapy treatments. Significance value for MC-Nemar’s test helped us in making statistical decision.
In our study, the significance value of NC-Nemar’s test was less than the predetermined significance level of 0.05, we concluded that the difference between the data before and after Physical Therapy were significant.

In patients with anterior disc displacement (ADD) problem, due to increase in soft tissue thickness in centric occlusion (CO), the disc is placed in an anterior and medial aspect of the joint. In the bilateral type of this disorder, during mouth opening, the chin is deviated toward the defective side. This is because in the unilateral type, only in the defective side, disc is anteriorly displaced and the function of lateral pterygoid muscle in this side is delayed in comparison with the normal side and because bilateral lateral pterygoid muscle co-ordinated contraction is the cause of non-deviated mouth-opening, and in ADD patients this is not possible, during mouth opening because of earlier contraction of lateral pterygoid muscle on the normal side, chin is deviated to the defective side.

The temporal muscle has an anterior and a posterior part. The anterior belly has a function in maximum dental intercuspidation and at the end of the mouth closing: therefore it is sensitive to interferences and besides that it stabilizes the jaw in protrusive movements. The posterior belly causes retraction of jaw when it is activated bilaterally, while it in unilateral contractions causes lateral movement of the jaw.
The medial pterygoid muscle leads to mouth closing and causes protrusive movements.
Mouth-opening muscles consist of the lateral pterygoid muscles and the digastric posterior muscle.

Temporomandibular dysfunction (TMD) is a generic term that involves the temporomandibular joint (TMJ) and is almost always coincident with disorders like pain in jaw, head and neck region. Symptoms such as headache, otalgia and disorientation, limitated jaw movements, discdisplacement or regular disarticulation of jaw and a presence of a bulge or asymmetry in the masticatory muscles are common. It is estimated that 75% of American population have one or more of the TMD signs and symptoms and about 5 to 10 percent of them need professional treatment.(2-3)

Hypermobility (Subluxation) of temporomandibular joint is established when dysfunction of suprahyoid and infrahyoid muscles and also ligaments axists. Dysfunction of these muscles is due to lack of normal bite.
In a joint suffering from hypermobility (Subluxation),condyle and disc start their movement from Glenoid Fossa and move forward along the slope of Articular Eminence and pass the Articular Eminence and are placed anterior to the Eminence. At this state, the amount of mouth opening is more than a normal joint.When condyle accompanied by disc are passing the Eminence, a click sound is heard.

In the majority of cases, the jaw articulation headaches have a combination of the above cases. These headaches may be continuous and daily. There are some patients suffering these headaches each day and each hour of the day. Never ever have they passed, a day free of these headaches.

To understand the importance of jaw articulation (Temporo mandibular joint = T.M.J) diseases we should make a statistical comparison with those suffering from systemic diseases of body. According to the cases recorded in European countries 30-40 percent of the population are suffering from one of these diseases. And 50 percent of these people require medical care. If we consider 30 percent of Iranian population - or 18 million people - suffer one of the jaw articulation diseases, almost 9 million seriously require medical care.
